COVID-19 Vaccinations

Ulster County residents can sign-up on Vaccine Pre-Registration Form to be put into their database for contacting when appointments become available at their point of distribution sites.

The purpose of this form is to gather contact information for individuals wishing to receive the COVID-19 vaccine as it becomes available.  Filling out this form does not sign you up for a vaccine appointment. After you fill out this form, your information will be added to a County mailing list, and you will receive updates about vaccine availability, your eligibility, and how to schedule an appointment for vaccination.

The local NYS site is taking appointments in New Paltz, at the Ulster County Fairgrounds. You can sign up on the NYS COVID-19 Vaccine Form.
